​Position: Licensed Practical Nurse (LPN)

 

The Licensed Practical Nurse (LPN) provides clinical nursing services to home care clients, ensuring high-quality, individualized care.

Job Responsibilities:

1. Conducts focused nursing assessments to determine appropriate interventions for maintaining or improving client health.

2. Administers interventions for clients experiencing potentially life-threatening instability.

3. Delivers nursing care to ventilator-dependent clients.

4. Provides in-home nursing visits and direct care as directed by the Registered Nurse (RN).

5. Prepares and administers medications according to the medication management program.

6. Oversees delegated staff to ensure adherence to the care plan and proper task performance by:

  •  Reinforcing teaching protocols established by the RN.

  • Addressing staff questions as needed.

  • Referring issues to the RN when necessary.

7. Coordinates medical appointments for clients, including following up on new orders or changes after visits, under RN direction.

8. Accepts and implements physician orders with guidance from the RN.

9. Handles phone calls from healthcare providers regarding clients, consulting with the RN as necessary.

10. Accurately documents client care in clinical records as required.

11. Communicates with families, responsible parties, case managers, and other healthcare professionals regarding client with direction from the RN as needed.

12. Is aware of and uses good safety practices, including but not limited to:

          a. Body mechanics

          b. Falls prevention

          c. Reporting of defective equipment or hazards

          d. Wiping spills or wet floors immediately​

          e. Using standard precautions and appropriate infection control measures

          f. Knowledge of emergency procedures

          g. Reporting incidents/incidents to the Director of Nursing or Administrator on the day of occurrence

13. Participates in care conferences with other members of the health care team

14. maintains absolute confidentiality of all information pertaining to clients, families, and employees

15. Promotes a safe and comfortable environment for the client and family

16. Demonstrates excellent written and verbal communication skills

17. Recognizes and manages own stresses, which may affect work performance

18. Participates in team meetings

19. Participates in in-service educational programs, as required by state regulations

20. Performs additional duties as assigned by the Director of Nursing or Administrator

21. Adheres to all agency policies

Experience/Job Skills Requirements:

  • High School Diploma or equivalent.

  • Graduate of a practical nursing program and licensed as a practical nurse in the State of Minnesota.

  • Experience in home care or assisted living is preferred.

  • Must be able to read, write, comprehend, and follow instructions effectively

  • Ability to function independently.

  • Must maintain good health and emotional stability.

  • Demonstrates a strong commitment to providing excellent service.

Working Condition Requirements:

Physical Demands: (Occasionally) refers to 1-2 hours of an 8-hour workday.

(Frequently) refers to 2.5 to 5.5 hours of an 8-hour workday and continually means to 6 to 8 hours of an 8-hour workday.
