Job Summary:
This position is responsible for ensuring that the healthcare needs of all clients are met. The Registered Nurse (RN) will also ensure compliance with Comprehensive Home Care regulations. Working collaboratively with the Director of Nursing, the RN will oversee service delivery to clients and supervise staff.
Qualifications:

Must be a Registered Nurse (RN) licensed in the State of Minnesota with a current and unrestricted license.

A valid driver’s license is required if driving is necessary.

Minimum of one year of experience in a related field.
Specialized Knowledge and Abilities:
The ideal candidate must:

Demonstrate sensitivity and an aptitude for working with the elderly.

Possess strong communication skills for interactions with staff, clients, family members, and the Director of Health Services.

Exhibit leadership and supervisory abilities.

Effectively prioritize and organize tasks efficiently.

Be available to work irregular hours, including occasional weekends and evenings.
Essential Job Functions and Tasks:
Primary Job Function: Provide Support and Address Program and Service Needs
Key Responsibilities:

Maintain client files and ensure proper documentation.

Conduct ongoing client assessments to evaluate and meet their needs.

Ensure clients continue living safely within their environment, reassessing and recommending alternative placements if necessary.

Communicate service plans for each client to the appropriate staff and ensure proper implementation.

Notify staff of any changes in client needs or conditions.
-
Maintain clear and consistent communication with clients, families, and staff.
-
Facilitate coordination of services with other healthcare providers as needed.
-
Be available for on-call medical inquiries and respond to emergencies via phone.
-
Develop and maintain a positive working relationship with staff, and other Gbaleah Homecare's staff including providing backup to other staff as needed.

Articulate information regarding clients to the team.

Provide proactive, constructive participation in staff meetings, committees, etc.

Assists other departments with special events and family functions.

Be compassionate and work with tact and ethical awareness.

Be flexible and adaptable to changing situations.

Responds appropriately to safety hazards, fire drills and other emergency situations.

Reports injuries for self, staff, clients or visitors immediately to Director of Health Services.

Keep Director of Health Services informed and involved and advised of needs and problems.
